Send the photocopy with the following documents to the address given below:
A)Two Passport photosof Donor
B) Donor’s Aadhaar Xerox
C)Two Family members /Witness Aadhaar Xerox

2.SOME PRE-REQUISITES
A)After death immediately need to inform the HOD of Anatomy/ institutionas mentioned above
B) Body has to be kept in ice box ( Freezer Box) till it reachesthe anatomy department (within 6 – 8 hours of death)
C) If informed, hospital will arrange ambulance to transport the body to our institution irrespective of holidays.
D) Death certificate issued by the registered medical Practitioner is mandatory to avoid legal complications
